# Tellescope

> Send completed intake answers to a Tellescope Custom Form

Use this destination when the intake should land in a Tellescope Custom Form. Setup for Forms
and Protocols is in [Overview & setup](/sdks/protocols/overview); below is what to pass when
calling **`send()`** with destination **`tellescope`**.

  Use **`true`** for lead capture (the Tellescope submission stays open). Use **`false`** or omit to
  close the submission — for example after payment.
